"What was old is now new again. The idea of the old-time country doc where you could directly pay for services is the heart of direct primary care."
I am a family medicine physician with a unique background in chaplaincy, prevention, public health, and community care. After spending a summer as an interfaith hospital chaplain, I deeply appreciate the power of sitting in someone's presence, fully listening, and becoming a true partner in your life's journey.
Why family medicine? At age 12, my physician cousin diagnosed my appendicitis—that rapid response averted disaster and sparked a lifelong interest in medicine. Many do not have access to a doc in their family. This model helps with that. Family medicine embodies full-blown curiosity about what makes people thrive.
My family and I have lived in Sylvania for the past 4 years - just outside of downtown. I want to see all of us age well and prevent many illnesses. We all are invested in our community and neighborhood.

The DPC Advantage: Real Cost Savings for Individuals
Understanding the true cost of healthcare involves more than just insurance premiums. This analysis reveals how Direct Primary Care (DPC) significantly reduces overall healthcare expenses and out-of-pocket costs for a single 35-year-old individual across various scenarios.
Routine Care Scenario: Managing Day-to-Day Needs
Let's compare the annual healthcare costs for a single 35-year-old individual who has two urgent care visits, two primary care visits, and regular medication throughout the year.
Without DPC
With DPC Membership
In this routine care scenario, DPC membership dramatically reduces the "out of pocket max" (green portion) and the "deductible still left over" (yellow) sections. By covering routine services, DPC acts as a buffer against unexpected bills, offering clear savings compared to traditional insurance models where co-pays and deductibles quickly add up.
Imaging Scenario: The Cash Pay Advantage
Now, consider the same single 35-year-old individual, but with the added need for one abdominal CT scan during the year.
Without DPC
With DPC Membership
Cash Pay Imaging: A Game Changer
This scenario highlights one of DPC's most significant advantages: access to cash-pay pricing for imaging. While DPC doesn't cover procedures like CT scans directly, DPC physicians can order imaging directly at cash-pay facilities, bypassing the typical insurance markup.
An abdominal CT through insurance might cost $2,000-3,000 (applied to your deductible), but cash pay pricing can be as low as $400-800. This substantial difference helps keep the "deductible still left over" (yellow section) dramatically lower for DPC members, as shown in the graphs. This applies to CT scans, MRIs, X-rays, and ultrasounds, often leading to 50-70% lower costs than insurance-negotiated rates.
Key Takeaways for Individuals
Dramatic Out-of-Pocket Reduction
DPC membership greatly reduces unexpected out of pocket costs in routine care scenarios, offering predictability and significant savings on common visits and medications.
Unlocking Cash Pay Imaging
DPC provides access to dramatically lower cash-pay rates for imaging (CT, MRI, X-rays), often 50-70% less than insurance rates. This keeps your "deductible still left over" (yellow) much lower.
Best Value with High-Deductible Plans
DPC pairs perfectly with lower-premium, high-deductible insurance plans (e.g., Bronze, Silver). It covers your routine needs affordably while your insurance protects against major medical events, maximizing your savings.
Predictable & Transparent Costs
DPC converts variable healthcare expenses into a manageable fixed monthly cost, bringing financial peace of mind and allowing for better budgeting without unexpected medical bills for primary care.
Family Healthcare: Maximizing Value with DPC
Families often face higher healthcare utilization due to the varied needs of multiple members, from routine check-ups for children to managing chronic conditions for adults. Direct Primary Care (DPC) provides even greater value for families managing multiple members' healthcare needs by offering comprehensive, accessible, and cost-effective primary care solutions.
Routine Family Care Scenario
Let's compare the annual healthcare costs for a family of 4, assuming 5 urgent care visits, 6 primary care visits, and 1 medication for the whole family during the year.
Without DPC
With DPC Membership
By consolidating primary care costs into a fixed membership fee, families gain predictability and substantial savings compared to traditional insurance models where co-pays and deductibles quickly accumulate across multiple family members. Yes, it is a bit more expensive, but the value of one provider, with significantly improved access remains.
Family Imaging Scenario - Cash Pay Savings Multiplied
Now, consider the same family of 4, but with the added need for one abdominal CT scan during the year.

Cash Pay Imaging: The case for Families
This scenario highlights how cash-pay imaging benefits are amplified for families. When any family member needs imaging (CT, MRI, X-ray, ultrasound), DPC provides access to 50-70% lower cash-pay rates. For example: a family CT scan through insurance might cost $2,000-3,000 (applied toward your deductible), but cash pay pricing can be as low as $400-800. This substantial difference keeps deductibles lower across the board, as shown in the graphs. Over time with multiple family members and varying healthcare needs, these savings compound significantly. DPC physicians order directly at cash-pay facilities, avoiding insurance markup entirely.
Key Takeaways for Families
Amplified Savings for Multiple Members
With 4 family members, routine care costs multiply quickly without DPC. DPC's flat family rate covers everyone's primary care needs, providing significant savings.
Cash Pay Imaging: Family-Wide Benefit
Any family member needing imaging gets 50-70% savings through cash-pay pricing, keeping deductibles lower across the board and reducing overall family medical expenses.
Perfect Pairing with High-Deductible Plans
The "other option" shows dramatic savings - families can choose lower-premium catastrophic coverage while DPC handles most routine care affordably, maximizing value.
Predictable Family Budget
Convert unpredictable family healthcare costs into one manageable monthly fee, with transparent pricing for the whole household, bringing financial peace of mind.

No, Medicare does not currently cover DPC memberships. However, many Medicare patients find DPC an excellent complement to their Medicare coverage. At this time Dr. Pomrenke is not accepting Medicare.
